Job Overview:
We are seeking a dedicated and compassionate Social Worker to join our team at a hospital-based facility in Cumming, GA. This position is a full-time, 8-week contract opportunity offering guaranteed 40 hours per week with an evening shift schedule.

Monday to Friday, 3:30 PM – 12:00
AM Key Responsibilities:
Provide social work services to patients in an acute hospital setting, offering emotional, psychological, and social support to individuals and their families. Conduct assessments to identify the social, emotional, and financial needs of patients. Develop and implement individualized care plans, collaborating with healthcare providers, including physicians, nurses, and other interdisciplinary team members. Assist patients and families in accessing community resources, financial support, and other services. Provide crisis intervention, including counseling and short-term psychotherapy for patients experiencing psychological distress. Facilitate discharge planning, including coordinating with community resources and services to ensure smooth transitions. Maintain accurate documentation of patient interactions, progress notes, and assessments in compliance with hospital policies and legal requirements. Participate in case reviews, team meetings, and other interdisciplinary activities to ensure holistic care for patients. Adhere to all ethical standards, patient confidentiality guidelines, and hospital policies.
Qualifications:
Licensing:
Must hold an active Licensed Clinical Social Worker (LCSW) certification in the state of Georgia.
Experience:
Minimum of 2 years of experience in Acute Hospital-Based Social Work.
Certifications:
BLS (Basic Life Support) certification required.
