PURPOSE OF POSITION
Community Coordinator (Georgia THRIVe) The Community Coordinator is a key role between Easter Seals North Georgia and Georgia THRIVe to address systematic gaps for infant and toddlers involved, or at risk of involvement, within the child welfare system. The Community Coordinator will collaborate with families, Gwinnett County Judicial and Child Welfare, and community programs to implement the Safe Babies Approach and evaluate the local Infant Toddler Court Program initiative.
ESSENTIAL DUTIES AND RESPONSIBILITIES

ESSENTIAL FUNCTIONS
Convene community partners and stakeholders to fully implement the Safe Babies Approach Coordinate collaboration among community stakeholders Facilitate Active Community Team (ACT) meetings Meet the ongoing educational and training needs of local partners and stakeholders Promote systems change for the prenatal to three populations and their families through cross-sector collaboration Recruit and enroll families into the Gwinnett THRIVe Program Facilitate service coordination for program participants Meet with families weekly to assist with achieving their goals and maintain regular contact Facilitate Family THRIVe meetings with families, attorneys, child welfare, and local service providers Attend court hearings and provide updates on family progress and challenges Assist with the development and maintenance of family prevention plans Ensure documentation, reports, and records are completed and submitted according to requirements Participate in nationwide Community of Practice calls with other Community Coordinators Participate in regular meetings with the Georgia THRIVe state leadership team and the national Zero to Three leadership team Travel within the county for court, family visits, community resources, and required program trainings and conferences

QUALIFICATIONS
Community Coordinator (Georgia THRIVe) Ability to work collaboratively with a multidisciplinary group across agencies and with a team across multiple agencies throughout the county and the state Bachelor Degree in social work, child development, sociology or relative field, preferred. Familiarity with Gwinnett County community and local resources including early intervention, medical care, education, mental health, and behavioral health Understanding of issues related to child abuse and neglect, intimate partner violence, substance use disorders, mental illness, and trauma Experience providing trauma-informed services to families Highly collaborative management style Excellent organizational and project management skills Ability to work independently and as part of a team Performs other duties as assigned.
