When a person is facing a serious illness, it affects that person and their loved ones in many ways. While social workers can help find services or items to meet concrete needs, that is not the primary or only role of the hospice social worker. Often a patient’s illness triggers challenges with finances, insurance, needs for community services, funeral planning or legal issues. Other less tangible issues patients and families struggle with relate to caregiver stress, juggling multiple roles, or just needing a safe third party to talk to. Social workers frequently provide support and counseling for patients and for caregivers.
JOB SUMMARY
To deliver varied social work services to Hospice patients and their families whether in their home or inpatient facility setting. To provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services and community education, outreach and referral. The Hospice social worker is an integral part of the Hospice IDG. Supervision is provided for an LMSW working toward the LCSW.
A Hospice Social Worker must:
1. Have an MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE) One year of experience in a health care setting is preferred. Medicare knowledge is preferred.
