MSW/Case Manager Heritage Care Place, LLC Stockbridge, GA 30281 • Hybrid work Job Summary We are seeking a dedicated and compassionate Social Worker/ Case Manager to join our team. The ideal candidate will play a crucial role in providing comprehensive support to clients, ensuring their needs are met through effective case management and advocacy. This position involves working closely with individuals in various settings, including healthcare facilities and community organizations, to assess their needs and develop tailored care plans. Duties Conduct thorough patient assessments to determine individual needs and develop appropriate care plans. Provide clinical counseling and support to clients, focusing on behavioral health issues. Maintain accurate medical records in compliance with HIPAA regulations. Collaborate with healthcare providers, social workers, and other professionals to coordinate services for clients. Facilitate group therapy sessions and support groups as needed. Monitor client progress and adjust care plans accordingly to ensure optimal outcomes. Advocate for clients’ needs within the healthcare system and community resources. Qualifications Master’s degree in Social Work Proven experience in case management or related field is highly desirable. Strong skills in patient assessment, clinical counseling, and behavioral health practices. Familiarity with medical records management and HIPAA compliance. Excellent communication and interpersonal skills to effectively interact with clients and multidisciplinary teams. Ability to work independently as well as collaboratively within a team environment.
