How you will change lives As a Social Worker at US Renal Care, you will be an integral partof a cross-functional
- team, working to help patients living withkidney disease achieve maximum social functioning and psychologicaladjustment to dialysis treatment and rehabilitation. What you will be doing Advocate & Support.
- You will be part of an interdisciplinary teamworking to ensure
- patients receive the best care, includingconducting all required patient assessment and
- care planningactivities such as assessing new patient psychosocial needs andcompleting the KDQOL in accordance with company policy and allstate/CMS regulations. You will identify and counsel psychosocialissues and provide
- patient and family education. As an advocatefor your patients’ needs, you will coordinate communities ofsupport for patients and their families, identify social agenciesand other resources (e.g.,
- financial/funding), provideinformation and referrals, coordinate transient arrangements, andrepresent your patient as needed with appropriate local, state, andfederal agencies.
Teamwork. As part of the interdisciplinary clinic team, promoteteamwork, educate staff, and provide training around patientpsychosocial care. You will participate in all required continuingeducation and staff meetings. You will collaborate with the MedicalDirector and physicians and maintain positive relationships witharea hospitals, agencies, vendors, and the community. Safety & Quality.
- You will help with clinical and operationalprocesses to improve patient health and minimize missed treatmentsand hospitalizations, achieving target goals for patient outcomes.
You will also participate in monthly Quality Assessment andPerformance Improvement (QAPI) activities and ensure compliancewith federal, state, and local laws and regulations.
What we’relooking for Master’s Degree in Social Work accredited by the Council ofSocial Work Education (CSWE). Current licensure (in good standing) in applicable state isrequired unless employed in the state of AZ, PA or Guam. Must meetany practice requirement(s) for the applicable state. Demonstrated working knowledge of the English language andability to communicate verbally and in writing. Basic computer skills, including Microsoft Office (Word, Excel,Outlook). Proficiency in all USRC clinical applications required within90 days of hire. Preferred Previous experience in providing social services to dialysispatients preferred. Other Requirements Must meet any practice requirement(s) for the applicablestate. Additional license requirements may be applicable dependingupon state. Are you ready to make a difference?
