Job Description

Service Coordinator – Muli-Family (Resident Support) Phase Housing Management LLC Columbus, GA 31901 Job Overview We are seeking a dedicated and compassionate Service Coordinator to join our team. The Service Coordinator serves as a vital resource for senior residents, connecting them with supportive services that allow them to maintain their independence and quality of life. This role involves building relationships with residents, identifying needs, coordinating services, and promoting wellness and community engagement.

Key Responsibilities:

Resident Support & Referrals Identify needs of residents through regular outreach and individual assessments. Connect residents with appropriate services including home health care, transportation, meals, financial assistance, mental health, and aging services. Maintain current knowledge of local community resources and service providers

Case Management:

Maintain confidential resident files and case notes in accordance with HUD/LIHTC or community guidelines. Monitor and follow up on referrals and services provided to ensure continuity of care. Track service utilization and resident outcomes for reporting purposes.

Programming & Education:

Coordinate educational workshops on topics like wellness, nutrition, fall prevention, financial literacy, and benefits enrollment. Organize and facilitate group activities and social events to encourage resident engagement and reduce isolation.

Advocacy & Compliance:

Advocate for residents’ needs while respecting their autonomy and decision-making. Ensure compliance with all

HUD/LIHTC

requirements and service coordination policies. Assist with annual reporting and audits as needed.

Qualifications:

Associate’s or Bachelor’s degree in Social Work, Human Services, Gerontology, or related field preferred. 2+ years of experience working with seniors or in social service coordination. Knowledge of aging-in-place strategies, Medicare/Medicaid, SNAP, and senior housing programs. Excellent interpersonal, organizational, and communication skills. Proficiency with Microsoft Office and case management databases. Ability to maintain confidentiality and professionalism.

Preferred:

HUD Service Coordinator Training or equivalent within 12 months of hire. Familiarity with fair housing regulations and supportive service models.
