Job Description:
Loving Arms Hospice is seeking a dedicated and empathetic Hospice Social Worker to join our compassionate team. The Social Worker will play a vital role in assessing and addressing the psychosocial needs of hospice patients and their families, providing counseling, support, and resource linkage throughout the end-of-life journey and into bereavement. This position requires a strong understanding of grief and loss, excellent communication skills, and the ability to work effectively as part of an interdisciplinary team.
Responsibilities:
Conduct comprehensive psychosocial assessments of patients and families to identify needs, strengths, and coping mechanisms. Provide professional counseling and emotional support to patients and families coping with terminal illness, grief, and loss, including pre-bereavement and anticipatory grief. Assist patients and families with navigating complex healthcare systems, understanding insurance benefits (e.g., Medicare, Medicaid), and accessing community resources. Facilitate discussions around advance care planning, including living wills, power of attorney, and funeral arrangements, ensuring patient wishes are respected. Collaborate with the interdisciplinary team (nurses, physicians, CNAs, chaplains, volunteers) to develop and implement individualized plans of care. Provide crisis intervention and support as needed. Advocate for patient and family rights and needs. Document all patient and family interactions, assessments, and interventions accurately and in a timely manner according to regulatory guidelines and agency policies. Participate in interdisciplinary team meetings to ensure coordinated and holistic patient care. Offer bereavement support services to families after the patient’s passing. Educate patients, families, and community members on hospice philosophy and services.
Qualifications:
Master’s Degree in Social Work (MSW) from an accredited school of social work. Current and unrestricted Social Work licensure (LMSW or LCSW) in the State of Georgia. Strong understanding of the psychosocial aspects of terminal illness, death, dying, and bereavement. Excellent assessment, counseling, crisis intervention, and communication skills (verbal and written). Ability to work independently and manage a caseload while effectively collaborating with an interdisciplinary team. Compassionate, empathetic, and patient-centered approach to care. Knowledge of community resources and ability to make appropriate referrals. Proficiency in electronic health record (EHR) systems and Microsoft Office Suite. Valid driver’s license, reliable transportation, and current auto insurance. Must be able to pass a background check and drug screen.
