How you will change lives As a Social Worker at US Renal Care,you will be an integral part of a cross-functional team, working tohelp patients living with kidney disease achieve maximum socialfunctioning and psychological adjustment to dialysis treatment andrehabilitation. What you will be doing Advocate & Support. You willbe part of an interdisciplinary team working to ensure patientsreceive the best care, including conducting all required patientassessment and care planning activities such as assessing newpatient psychosocial needs and completing the KDQOL in accordancewith company policy and all state/CMS regulations. You willidentify and counsel psychosocial issues and provide patient andfamily education. As an advocate for your patients’ needs, you willcoordinate communities of support for patients and their families,identify social agencies and other resources (e.g.,financial/funding), provide information and referrals, coordinatetransient arrangements, and represent your patient as needed withappropriate local, state, and federal agencies. Teamwork. As partof the interdisciplinary clinic team, promote teamwork, educatestaff, and provide training around patient psychosocial care. Youwill participate in all required continuing education and staffmeetings. You will collaborate with the Medical Director andphysicians and maintain positive relationships with area hospitals,agencies, vendors, and the community. Safety & Quality. You willhelp with clinical and operational processes to improve patienthealth and minimize missed treatments and hospitalizations,achieving target goals for patient outcomes. You will alsoparticipate in monthly Quality Assessment and PerformanceImprovement (QAPI) activities and ensure compliance with federal,state, and local laws and regulations. What we’re looking for Master’s Degree in Social Work accredited by the Council ofSocial Work Education (CSWE). Current licensure (in good standing) in applicable state isrequired unless employed in the state of AZ, PA or Guam. Must meetany practice requirement(s) for the applicable state. Demonstrated working knowledge of the English language andability to communicate verbally and in writing. Basic computer skills, including Microsoft Office (Word, Excel,Outlook). Proficiency in all USRC clinical applications required within90 days of hire. This job is located in Lafayette, Indiana – and will travelsome to Kokomo, IN Preferred Previous experience in providing social services to dialysispatients preferred.
