Job Summary:
Provides medical social work services to patients and families in the acute care setting as well as to the Emergency Services department. This includes psychosocial assessments, discharge planning, linkage to community resources, collaboration with other treatment team members, and/or supportive counseling and crisis intervention. 1. Clinical Practice and Care Management a. Conducts high quality patient and family assessments in order to identify patient and family needs and long-term discharge needs in a manner sensitive to the overall health and well-being of the patient. b. Provides high quality individual, family/group supportive counseling and crisis intervention in a sensitive, caring, and professional manner. c. Coordinates referrals to long term care facilities. This includes counseling patients/families regarding need for placement and ensuring that necessary paperwork is available to the facility. 2. Documentation a. Explains and discusses the living will and durable power of attorney to patients and family members in a sensitive and caring manner in accordance with established hospital policy and procedure. b. Follows policies and procedures regarding discharge planning, psychosocial assessments and chart documentation. 3. Communication, Partnership and Collaboration a. Coordinates services for home care, equipment and/or community resources following discharge. b. Coordinates referrals for hospital-to-hospital transfers and rehabilitation centers. c. Promotes the provision of compassionate health care services by communicating and interpreting patient and family concerns to staff members through discharge planning meeting, treatment team meeting and/or consultation. d. Observes and adheres to all departmental and hospital policies and procedures, and follows all safety, quality assurance, and infection control standards. e. Demonstrates professional judgment in reporting cases of suspected abuse or neglect while maintaining patient and family member confidentiality. f. Acts as an information and referral source by developing cooperative agreements with community agencies and services in an effort to meet patient needs. g. Works with other members of the treatment team to ensure discharge needs and timely discharges. 4. Professional Development a. Promotes the quality and efficiency of own performance through participation in staff educational programs, approved continuing education courses, and specialized skill training programs. b. Performs other duties as assigned or requested.
