Videos Duties Social Work :

This position serves as Senior Social Worker in the Mental Health Residential Recovery and Trement Program (MHRRTP) the Alexandria VA Health Care System (AVAHCS) main campus. Duties performed include, but are not limited to the following: Collabores with the other members of the interdisciplinary trement team in the provision of comprehensive mental health services to Veterans, ensures equity of access, service, and benefits to this populion and ensures the care provided is of the highest quality. In conjunction with the healthcare team, identifies and assesses risk, safety, supports and implements approprie interventions/trement for those who are experiencing abuse, neglect, exploition or are experiencing domestic or intime partner violence. Adheres to VA and manded reporter requirements. Advises and collabores with interdisciplinary teams throughout the medical center on abuse, neglect, exploition and intime partner violence issues. Possesses advanced knowledge of and mastery of theories and modalities used in the specialized trement of SUD and co-occurring conditions. Ability to incorpore complex multiple causion in differential diagnosis and trement of veteran pients, including making psychosocial and psychiric diagnoses within approved clinical privileges. Provides Advanced Directive informion and educion including but not limited to the following: Durable Power of Attorney for Health Care, Living Will, Health Care Directive, and/or trement preferences document to Veteran and families. Also assists the Veteran in preparing such documents, as requested. Assists the interdisciplinary team with identifying approprie decision makers if Veteran is unable to make own decision (Next of Kin, HCPOA, Legal Guardian). Performs comprehensive suicide risk screenings and assessments and develops and integres safety planning and safety goals into the Veteran’s collaborive, individualized trement plan. Ability to determine priority for services and provide specialized trement service. Provides case management services for Veterans in residential trement AVAHCS. Completes post discharge follow up appointment calls for those discharged from residential care AVAHCS and other VA facilities. Provides clinical services on weekends as part of a rotion with other clinical providers.
