Your Mission :
Champion Recovery, Empower Patients As an Inpatient Rehabilitation Social Worker with a case management focus, you are both a clinical navigator and a patient advocate. You will lead the psychosocial assessment and discharge planning process, ensuring that all patients care plan is not only clinically sound but also tailored to their unique goals and circumstances. Your leadership will be vital in coordinating interdisciplinary care, facilitating communication, and driving outcomes that matter. Key Responsibilities Comprehensive Psychosocial Assessment Conduct in depth psychosocial evaluations for all patients within 72 hours of admission, identifying strengths, challenges, and support systems. Collaborate with patients and families to establish individualized goals for rehabilitation and discharge. Care Coordination & Team Leadership Serve as the primary point of contact for patients, families, and the interdisciplinary team regarding care planning, progress, and discharge needs. Lead and document regular team conferences, synthesizing input from therapy, nursing, and medical staff to ensure unified, patient centered care. Facilitate family conferences and provide ongoing counseling for psychological, financial, or family issues related to disability or recovery. Discharge Planning & Community Integration Develop, coordinate, and implement comprehensive discharge plans, arranging for community services, equipment, and follow up care as needed. Ensure smooth care transitions by communicating discharge plans to families, payors, and receiving facilities, and by providing post discharge follow up. • Regulatory & Quality Leadership • Ensure compliance with Medicare (CMS) guidelines, including documentation, utilization review, and the QIO appeals process. Participate in quality improvement initiatives, hospital committees, and community education programs to advance patient outcomes and organizational excellence. Educate the interdisciplinary team on cost containment, benefits plans, and evidence based practices. Patient Advocacy & Ethical Stewardship Champion ethical and legal standards in patient care delivery, ensuring that every patient rights and needs are respected.
