Job Description

As a member of the Adult Family Care/ Shared living team, Case managers,use their knowledge of social work, people skills, & community servicesto facilitate & maintain family homes. Case Managers use their uniqueprofessional skills to ensure a higher level of care of the clients theyserve.

ESSENTIAL JOB FUNCTIONS

Provides case management services withinestablished departmental policies & procedures. Assesses & providespsychosocial evaluations of potential participants & caregivers.

Conducts monthly/ bi-monthly site visits. Trains caregivers, conductsannual home inspections & develops & updates safety plans, as needed.

Prepares & submits individual service plan (ISP) goals & assessesindividuals funded by the Department of Developmental Services (DDS).Assists program participants in securing & maintaining Mass Healthcoverage, social security entitlements, food stamps, & Section 8housing, as needed. Enters appropriate & timely documentation of caseactivity in participant?s records. Develops family care placement plansbased on social work practice with community health nurse; facilitatesmatching process with potential participants & host families. Providescasework/case management services to ongoing caseload including routinefollow-up services, discharge planning & crisis intervention. Maintainsprofessional growth & development through approved seminars, workshops ≺ofessional affiliations to keep abreast of the latest trends in thefield. Collaborates with other nursing staff, as needed. Serves asmandated reporter for suspected cases of abuse &/or neglect to theDisabled Persons Protection Commission (DPPC).

OTHER DUTIES &RESPONSIBILITIES

Performs additional duties, as directed.

PREPARATION,KNOWLEDGE, SKILLS & ABILITIES

Minimum 2 years prior experience workingwith elderly or disabled adults. Bachelor’s degree Attention to detail.

Valid driver’s license in the state of Massachusetts required.

Successful completion of all state & Agency mandated trainings requiredupon hire. Reliable vehicle required

SUPERVISORY RESPONSIBILITY

Nosupervisory responsibility.

WORKING CONDITIONS/PHYSICAL DEMANDS

Frequentsitting & walking. Ability to travel in a car for prolonged periods. Seecompetencies when applying.
