We are currently seeking a Supportive Case Manager to join our child care team and provide critical support, guidance, and casework to toddler, preschool, and school-age children and their families, especially those referred through the Department of Children and Families (DCF).

What You’ll Do:

As a Supportive Case Manager, you will play a key role in helping children and families navigate challenges and access the support they need. You will collaborate with internal staff and external agencies to ensure families receive coordinated care and support throughout their journey with the Y.

Key Responsibilities:

Manage intake and enrollment procedures for children referred by DCF Serve as a liaison between the YMCA, DCF, and the child’s support team Collaborate with external agencies to assess and evaluate individual needs Maintain accurate and confidential case documentation and files Develop and implement Individual Service Plans (ISPs) based on goals and observations Conduct assessments and monitor the child’s adjustment and progress Complete and share Progress Reports with

DCF, YMCA

leadership, and families as needed Act as a resource for YMCA staff , including guidance on mandated reporting, child abuse prevention, and behavior management Conduct weekly site visits and maintain open communication with all relevant parties Partner with YMCA staff to identify and support training needs related to the population served Participate in ongoing professional development and stay informed of best practices in case management

Qualifications:

Bachelor’s Degree in Human Services , Education , or a related field preferred ; OR at least 3 years of experience working with Supportive Contracts or at-risk families Strong understanding of child development, family dynamics, and trauma-informed care Reliable transportation required for travel between multiple YMCA sites Ability to complete work in a timely, professional , and confidential manner Must demonstrate good judgment, integrity, and alignment with the YMCA mission and values

Requirements:
