Union/Non-Union Duties and Responsibilities:
Provide intensive outreach and care management services to individuals with substance use disorders who are charged with criminal offenses or are facing civil commitment in the Holyoke/Chicopee District Courts. Help court-involved individuals address barriers to recovery through increased access to safe housing, employment, transportation, prescribed medication, and support networks. Coordinate care with physical and mental health care providers, public service agencies, community service organizations, courts, corrections, and social supports. Support applying for government benefits, identification, and community services. Provide temporary transportation to court hearings, peer support groups, essential medical and behavioral health appointments while transitioning to community-based transportation resources. • Assist individuals with educational and vocational assessment and placement. Assist individuals with recovery, wellness, and empowerment. Provide crisis intervention as needed. Maintain professional, accurate and timely progress notes, wellness plans and other required documentation. Attend court advisory committee meetings and trainings as required. Maintain strong working relationship with Recovery Coaches, community agencies, probation department, court officials and court employees. Perform other duties as assigned.
Minimum Qualifications:
Bachelor’s degree in social work, psychology, nursing, or related field. Valid driver’s license and automobile with insurance.
Skills, Knowledge and Prior Experience Required:
Knowledge of behavioral health and medical services, resources and supports for housing, vocational training and employment in the community An elevated level of skill in organization, time management, ability to multitask and work independently. Basic computer skills. Ability to communicate professionally and accurately. Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds, cultural competency. Experience in human services case management preferred. Fluency in Spanish, spoken and written, preferred.
Supervisory Relationships:
Reports to supervisor at employer (culturally specific organization).
