Our client is currently seeking a Licensed Social Worker to join their team based out in Lowell, MA! The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility’s Social Services Department. As a Social Worker, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. This job will have the following responsibilities: Assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs. Participate in discharge planning, resident assessments, and developing social care plans. Interview residents/families to obtain social history. Refer residents/families to appropriate community agencies when needed. Provide consultation to staff and community agencies on meeting residents’ social/emotional needs. Assist in obtaining resources from community agencies to meet residents’ needs. Ensure confidentiality of residents’ protected health information.
Qualifications & Requirements:
Bachelor’s degree in Social Work. 1 year of supervised experience in healthcare.
