TheElement Care Social Worker participates in the planning,implementation and evaluation of care plans that meet theobjectives, standards and policies of the PACE model of care. Thesocial worker demonstrates proficiency in providing traditionalsocial work services in a professional and respectful manner withthe goal of helping older adults live safely and comfortably intheir homes and communities for as long as they can. This positionis full time M-F 8am to 4pm.
RESPONSIBILTIES
- Participates on theIDT’s initial assessments, care planning and on-goingre-assessments of participant care.
- Attends IDT meetings;actively participates in team meetings by sharing pertinentinformation, providing follow up to assigned tasks and helps todevelop participant’s plan of care.
- Completes all assignedassessments
- (initial, semi-annual, annual, service request and/orsignificant event).
- Assesses the psychosocial needs of theparticipant and provides supportive counseling, workingcollaboratively with behavioral health providers.
- Facilitateshospital, rehabilitation and nursing home (NH) admissions anddischarges as determined by the Interdisciplinary Team. Ensuresthat PASRR documentation is completed for NH admission.
- Assistsin the conversion process of the participant from community tolong-term care. Works collaboratively with Medicaid Specialist,skilled nursing facility, and participant’s caregiver to completeconversion.
- Arranges and facilitates family meetings, as needed
- Refers participants and families to appropriate communityservices and acts as liaison and/or advocate with communityorganizations for participants.
- Maintains professional, accurateand timely social service documentation in the participants’medical records.
- Conducts participant council meetings asassigned.
- Works collaboratively with Director of Social Work andBehavioral Health provider to ensure guardianship is up to date.
Educates participant regarding health care proxy (HCP). Assistsparticipant in completion of HCP form.
- Works collaboratively withfiscal department to maintain participant insurance benefits andcompletes required documentation of fiscal information in themedical record.
- Reviews plan of care with participants, guardian,and/or activated health care proxy as assigned.
- Completeauthorizations for home care and other approved services timely andaccurately.
- Completes home and/or skilled nursing facility visitsto assess participant as indicated.
- Works collaboratively withPalliative care team; Assists with end of life planning asindicated.
- Provides timely communication to appropriate staffregarding the following: (disenrollment, conversion to long termcare, transfer of sites, participant and/or caregiver demographicchanges).
- Reports allegations of abuse to appropriate stateagency; provides support and resources to participant as he/shewill accept; completes required documentation.
- Ability to pass afit test. Position requires mask where seal is critical. Incumbentis required to not have facial hair that interferes with a tightseal of the respirator.
- Performs other duties as required.
- Frequent local travel.
Qualifications:
- Current Social Worklicensure in the Commonwealth of Massachusetts at the Masters level(L.I.C.S.W. or L.C.S.W.) required
- Minimum of 1 experience inSocial Work providing traditional Clinical or Case Managementservices with a geriatric population
- Current C. P. R.Certification or ability to become certified
- Treat allparticipants in a welcoming and professional manner.
- Strongverbal, written and listening skills with ability to multi-task ina fast pace environment
- Vietnamese and/or Khmer speaking but allMSW eligible candidates to apply.
