The Dialysis Social Worker isresponsible for assessing and responding to the psychosocial needsof our dialysis patients to ensure the best possible outcomes andimprove quality of life.
Schedule:
Fulltime Monday through Friday,weekends off

Pay range depending on dialysisexperience

Responsibilities What You Can Expect:
Provide patient support toaddress emotional, financial and lifestyle changes associated withchronic kidney disease Assist patients with utilizing communityresources for nutritional, emotional, transportation and housingsupport Advocate and assist patients with accessing healthcare andother benefits Educate patients and their families regardingend-stage renal disease treatment options, including dialysis,transplant and medical management Help patients compare treatmentoptions, empowering them to make informed decisions about theircare Coordinate translation services for patients and caregiversAssess family and support system and refer for additional servicesas needed Provide counseling services to patients and theirfamilies directed at helping them cope with kidney failure anddialysis, follow the treatment plan and achieve the patient’srehabilitation goals Develop and implement individualized careplans with interdisciplinary care team based on psychosocialassessment to help patients enjoy a better quality of lifeCollaborate with interdisciplinary care team to assess ongoingneeds and modify care plan as necessary Educate interdisciplinaryteam members about emotional aspects of dialysis Advocate forpatients at clinic, community and state/federal levels Serve asESRD Network liaison Engage in a data-driven quality improvementprocess to positively impact outcomes
