Position Summary:

The Social Workeris responsible to facilitate care along a continuum througheffective resource coordination to help patients achieve optimalhealth, access to care, and appropriate utilization of resources,balanced with the patient’s resources and right toself-determination. The individual in this position has overallresponsibility for to assess the patient for transition needsincluding identifying and assessing patients at risk forreadmission. Conducts complex psycho-social assessment andintervention to promote timely throughput, safe discharge, andprevent avoidable readmissions. This position integrates nationalstandards for case management scope of services including:

Transition Management promoting appropriate length of stay,readmission prevention, and patient satisfaction; Care Coordinationby demonstrating throughput efficiency while assuring care is theright sequence and at appropriate level of care. Compliance withstate and federal regulatory requirements, TJC accreditationstandards, and Tenet policy; Education provided to physicians,patients, families, and caregivers; and Leads a population ofpatients by service line and/or leads the team by being a resourceto Tenet performance standards. Responsibilities This individual’sresponsibility will include the following activities: a) complexpsycho-social transition planning assessment and reassessment andintervention, b) assistance with adoptions, abuse and neglectcases, including assessment, intervention and referral asappropriate to local, state and /or federal agencies, c) carecoordination, d) implementation or oversight of implementation ofthe transition plan, e) leading and/or facilitatingmulti-disciplinary patient care conferences including Complex CaseReview, f) making appropriate referrals to other departments, g )communicating with patients and families about the plan of care, h)collaborating with physicians, office staff, and ancillarydepartments, I) assuring patient education is completed to supportpost-acute needs, j) timely complete and concise documentation inCase Management system, k ) maintenance of accurate patientdemographic and insurance information, l) precepts new staffmembers and acts as a resource to all staff, m) facilitates TEMPOas needed, n) participates in department quality improvementinitiatives, and o) other duties as assigned.
