$21.63/hour Are you passionate about making a difference in the lives of youth and families? We are seeking a dedicated Case Manager I – Family Support to join our team, where you’ll provide crucial one-on-one support to parents and caregivers in a community setting. This role involves connecting families with resources, offering coaching, and helping build collaborative relationships with schools, service providers, and other community-based supports.

Responsibilities:

Provide family coaching, support, and resources to help connect families with their children’s care team, including schools, counselors, and healthcare providers. Develop and implement care plans, provide advocacy, and assist with service coordination and resource development. Maintain a caseload, ensuring documentation and service delivery are timely and meet best practice standards. Facilitate support groups, maintain the Family Resource Center (FRC) calendar, and engage volunteers for specialized groups. Collaborate with community partners and attend meetings and trainings to continue personal and professional development.

Qualifications:

High school diploma or GED required. 1-3 years of related experience. Valid driver’s license and reliable personal transportation. Experience or knowledge in youth and family casework, wrap-around care, and community resources. Ability to build effective relationships with clients, families, and providers. Strong communication, organizational, and problem-solving skills. Familiarity with child welfare, mental health, education, or juvenile justice systems is a plus.
