Job Summary:
- The Barron Center is a long-term care facility, owned and operated bythe City of Portland, and is Maines largest skilled nursing facility. Wehave an excellent reputation for fostering a caring and creative workenvironment, and now is your chance to have the job you always wanted

This position involves professional social work withlong-term care residents and their families, assisting in theiradjustment to the facility, and working as a team member to provide forand assure quality of life for facility residents. Qualified candidateswill have at least one year of experience in long-term care or withingeriatric care work, including knowledge of community resources. TheLicensed Social Worker designation is required for consideration andhire.

Duties & Responsibilities:
- Shares responsibility for telephone and walk-in referrals.
Provides appropriate community referrals.
Meets with residents and their family upon admission, determinespsychosocial needs, and formulates an appropriate care plan.
Visits residents on a regular and as-needed basis.
Maintains contact with families, involving them in residents care asappropriate.
Acts as an advocate for residents and their rights.
Participates in team conferences and psychiatric consultations.
Works as discharge planner arranging for services at time of discharge.
Responsible for preparing and maintaining social service documentationin accordance with State regulations.

Required Skills & Experience:
- Bachelors Degree in Social Work or a related field.
Licensed SocialWorker designation is required.
Minimum one year experience in long term care or geriatric care workExperience in family or group counselingWorking knowledge of social services system and community socialresourcesAbility to prepare complete and valid case recordsAbility to apply policies and procedures to specific cases.
Ability to assist families and individuals with social and economicplanning.
Ability to establish and maintain effective working relationships withother social workers, institutional and public officials, and othercommunity contacts

Expected Hours/Schedule:
- \Monday-Friday, 8am-4pm.

- The successful candidate will be subject to a pre-employment physicalexam. Offers of employment are contingent upon the completion of asatisfactory criminal background check.

Salary & Benefits:
- \This is a union position
- CEBA, Grade 15
- Starting pay rate is between$24.95
- $27.
