You’ll work closely with residents, their families, and ourinterdisciplinary team to provide comprehensive social workservices, ensuring emotional, social, and psychological needs aremet. Key Responsibilities Conduct psychosocial assessments and develop care plans. Provide counseling and support to residents and families. Facilitate group and individual counseling sessions. Advocate for residents’ rights and needs. Collaborate with healthcare professionals to coordinatecare. Maintain accurate documentation of social work activities. Participate in interdisciplinary team meetings. Assist residents in accessing community resources. Provide crisis intervention as needed. Oversee CNA interactions, fostering positive connections.

Requirements:

Job Requirements Licensed Social Worker with an active State of Mainelicense. Bachelor’s degree in a related field (social work, psychology,or rehabilitation). Experience working with individuals with intellectual anddevelopmental disabilities and in leadership role preferred. Strong communication and leadership skills.
