Your Job The Licensed Vocational Nurse (LVN) will operate as a Discharge Planning Coordinator. The LVN is responsible for the discharge of patients and following pre-established admission criteria in collabartion with RN Case Managers and other staff. You will work closely with the Social Worker and must be a naturally compassionate person. Qualifications Must have an active license for the state of Texas in good standing as either an LVN or an RN Responsibilities Ensure compliance with patient’s plan of care Collaborate with multiple departments Coordinate and execute timely discharge plans for a designated caseload of patients. Ensure a safe transition. Work closely with the Social Worker Report any issues to management.
