Full Job Description SUMMARY:
Under the direction of leadership, the Social Worker will document, support, and provide directions to Meritas Health patients on available community health resources. The Social Worker will lead resource documentation, referral practices, and recommendations across the healthcare system.
EXPERIENCE
5 years minimum experience as LBSW required, 7years preferred.
SPECIAL SKILLS
Demonstrates a high level of professionalism and knowledge of community resources. The ability to utilize critical thinking in advising patients of resources available to them to positively impact on their mental, emotional, and physical health. Expertise and proficiency in documentation and in use of electronic medical records. Must have an exceptional patient centered care mentality.
LICENSE/CERT
Required:
BSW-Bachelor of Science in Social Work
EDUCATION
Required:
Bachelors – Social Work (CSWE approved)
