SUMMARY:
Responsible for coordinating an interdisciplinary approach to deliver palliative healthcare services to patients. Plans, organizes, and implements Social Work services which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of the patient/families. Ensure appropriate decision makers are informed of and involved in treatment planning.
