The ideal candidate will provide essential support to patients and their families, helping them navigate the complexities of medical care and social services. This role involves working closely with individuals in various settings, including assisted living facilities, nursing homes, and hospitals, to promote their well-being and enhance their quality of life. Responsibilities Conduct comprehensive assessments of patients’ social, emotional, and environmental needs. Develop and implement individualized care plans in collaboration with healthcare professionals. Provide counseling and support to patients and families facing medical challenges. Facilitate group therapy sessions to address common issues among patients. Collect and analyze data related to patient outcomes and service effectiveness. Advocate for patients’ rights and access to necessary resources within the healthcare system. Educate patients and families about available community resources and services. Maintain accurate documentation of patient interactions, assessments, and progress notes. Stay informed about current trends in geriatrics, social work practices, and medical terminology. Qualifications A Master’s degree in Social Work (MSW) from an accredited institution is required. Valid state licensure as a clinical social worker or equivalent certification is preferred. Experience in special education, geriatrics, assisted living, or nursing home settings is highly desirable. Strong understanding of medical terminology and clinical research methodologies. Excellent data collection skills with the ability to analyze information effectively. Demonstrated ability to work collaboratively within a multidisciplinary team. Exceptional communication skills, both verbal and written, with a focus on empathy and active listening. Previous experience in group therapy or similar therapeutic environments is a plus.
