Job Description:

The Family Support Provider is responsible for developing asupport system for parents of children with serious emotionaldisorders. This is a Full-time, benefit eligible position.

ESSENTIAL FUNCTIONS – JOB SPECIFIC

Assist and coach family toincrease their knowledge and awareness of the child’s needs Assistthe family in building helpful relationships with child and familyteam members Link family with support groups and other communityresources Provide helpful materials such as videos, books, articlesor Internet address Provide moral support during crisis and othercritical periods Assist family with preparations for meetingsParticipate in other activities as defined by the treatment teamand family Appropriately submit required documentation; progressnotes completed collaboratively, within 24 hours Participate in thedevelopment of individualized service plans that wrap servicesaround the family needs Work collaboratively with the IHS and isgiven direction by the CPRC Youth Supervisor Driving is anessential function of this position Other duties as assigned
