Key Responsibilities Complete psychosocial assessments within 5 days of hospice admission. Identify emotional, social, financial, and environmental needs and plan appropriate interventions. Participate in Interdisciplinary Group (IDG) meetings to help develop and update each patient’s plan of care. Observe, record, and report changes in the patient’s emotional and social condition. Maintain accurate clinical records, including initial evaluations, care plans, and visit notes. Evaluate the effectiveness of social work interventions for patients and families. Coordinate weekly visit schedules with Case Managers. Attend IDG and Quality Assessment and Performance Improvement (QAPI) meetings and activities. Provide ongoing support to the care team if patient/family declines direct social work services. Assist with discharge planning as needed. Engage in professional development through training, workshops, and continuing education. Assess patients’ and families’ coping abilities related to end-of-life care. Serve as a consultant to the health care team on social, emotional, and environmental issues. Help patients and families use available community and family resources. Social Work Experience:

1 year of social work experience required for BSW; preferred for MSW. Hospice experience preferred.
