Social Work :

As the supervisor, the incumbent assists in planning, establishing, and implementing policies and procedures; developing individual or group goals and objectives as well as monitoring, opering, evaluing, coordining and overseeing the Social Work staff of the assigned programs. As an advanced practitioner, incumbent serves as the Social Work Supervisor for social workers in several medical and case management programs (Inpient Medicine/Surgery wards x4, Intensive Care Units

  • Renal and Oncology, Caregiver Support Program
  • CGS, and Move 2 VA (M2VA).

Incumbent is responsible for staffing, work assignments, and trement services provided. Duties include, but not limited to: Responsible to develop guidelines and performance expections for social work staff, observe workers’ performance, critique performance, provide informal feedback, and periodically evalue employee performance. Establishes guidelines and performance expections for staff, which are clearly communiced through the formal employee performance management system. Observes workers’ performance; demonstres and conducts work performance critiques. Provides informal feedback and periodically evalues employee performance. Performs work reled to the review, monitoring, analysis, planning and assessment of various programs. Responsible for key project activities reled to the planning, development, and implemention of the project component. A substantial portion of the workload directed by this position carried out one or more locions th are physically removed from the main unit under conditions th make day-to-day supervision difficult to administer. In addition to above duties incumbent provides direct clinical services, in each of the programs which are overseen when coverage is necessary and performs other duties as assigned. Provides administrive supervision to staff social workers assigned to inpient units, including performance evaluions, workload monitoring and disciplinary actions as needed. Develops and maintains standard opering procedures (SOPs) and quality assurance mechanisms for acute inpient social work services. Ensures compliance with local, VISN, and nional social work directives, policies, and VHA handbooks. Collabores with interdisciplinary teams, medical providers, nursing leadership, case management, and discharge planning teams to ensure high-quality pient care. Leads recruitment, onboarding, training, and mentoring of inpient social workers. Collects and analyzes da reled to inpient social work outcomes, discharge planning efficiency, readmission reduction, and care coordinion. Leads or participes in quality improvement (QI) and performance improvement (PI) projects targeting enhanced Veteran care and system efficiency. Ensures timely documention and compliance with all charting, workload capture, and consult response requirements. Assists in developing stregic plans and service line goals to expand or improve social work services in acute settings. Participes in hospital-wide committees such as Utilizion Management, Ethics Consults, Readmission Prevention, and Interdisciplinary Care Rounds. Ensures Veterans’ rights and preferences are upheld in all care decisions, with particular tention to vulnerable populions (e.g., homeless, aging, terminally ill). Licensure:

Loss of Licensure or Certificion:

Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certificion to remain qualified for employment. Loss of licensure or certificion will result in removal from the GS-0185 social worker series and may result in terminion of employment.

English Language Proficiency:

Candides must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).

Experience/Educion:

One year of experience equivalent to the GS-12 grade level. Experience must demonstre possession of advanced practice skills in a specialty area or in administrion demonstring progressively more professional competency and judgment. Licensure/Certificion:
