Line of BusinessSpring Valley Hospice Position TypePRNOur CompanySpring Valley HospiceOverviewThe Medical Social Worker provides social work services to patients in their place of residence, in accordance with the physician orders and plan of care under the supervision of the Clinical Manager or designee. ResponsibilitiesUnderstands and adheres to established company policies and proceduresParticipates in the coordination of company services with community resources and services, serving as liaison between patients or family/caregivers and community resourcesMaintains collaborative relationship with organization personnel to support patient careObserves confidentiality and safeguard patient informationServes as a resource for employees, patients, physicians, and other allied health providersDevelops a cooperative relationship and communicate effectively and professionally with healthcare providersMaintains current knowledge of Medicare Conditions of Participation for social work servicesParticipates in in-services and/or continuing education programsParticipates in patient care conferencesMaintains contact with community support groups and provide professional expertisePlans and coordinates all social services within the companyDocuments all patient/family services provided as required by company policyAssists physician and/or other care team members in understanding the significant social and emotional factors related to health problemsParticipates in the development of the plan of carePrepares clinical and progress notes in accordance with company policyParticipates with patient discharge planningSupervision of assistant personnel when required by lawOther duties as assigned and within scope of practiceQualificationsMaster’s or Doctoral degree from a school of social work accredited by the Council on Social Work EducationOne year of social work experience in a healthcare setting required, three to five years preferredOne year in home health care preferredValid license from the state practicing inCurrent CPR certificationBasic clinical skillsProfessional appearance and demeanorGeneral knowledge of computers and the ability to learn industry specific software applicationsExcellent oral, written, and interpersonal communication skillsExcellent organizational and time management skills with a keen attention to detailsAbility to work effectively in a team environmentAbility to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issuesAbility to learn and complete various educational goals during orientationAbout our Line of BusinessSpring Valley Hospice, an affiliate of BrightSpring Health Services, focuses on providing hospice care to local patients and their families living.
