We focus on supporting patients and families through the challenges of terminal illnesses by providing the highest standard of care, enabling them to live as fully and comfortably as possible. •Join our Team•We are currently expanding our team and are looking for a Licensed Social Worker. As a member of our team, your compassionate care, communication skills, and understanding of the psychosocial dynamics of illness, loss, and death will be instrumental in providing direct patient care during the final stages of life.
Key Responsibilities:
Assess the psychosocial status of patients and their families/caregivers related to the patient’s terminal illness and environment. Communicates any special needs to the interdisciplinary group. Evaluate family dynamics, caregiver abilities, volunteer and bereavement needs, etc., and provide interventions as necessary. Provides information to patients and their families/caregivers and community agencies.
Key Qualifications:
Meets state-specific guidelines, education, and licensure requirements. Meets personnel qualifications stated in §418.114(b)(3). Minimum of one (1) year of experience in health care; hospice experience preferred. Understands hospice philosophy and issues of death/dying. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR Certification. Must be licensed with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
