The Job Description General Statement of Duties Providing social work and case management services to clients in the In-Home Aide Level II program. Distinguishing Features of the Class Work involves evaluating the client’s situation and his/her ability to deal with it, developing a social history, psychosocial assessment, service plan and/or treatment plan and follow-up. Services may include basic counseling, support, and reinforcement in areas such as death and dying, medical, health, or behavioral issues. The worker addresses such problems as adjustment to illness or disability, placement into an institution, rest home or nursing facility; financial concerns; socialization issues of the socially handicapped or those debilitated by age or illness. This position is home-based and the hourly rate is inclusive of all travel and home office expenses. Recruitment and Selection Guidelines Essential Duties and Tasks Conducts independent annual and quarterly assessments to evaluate the effect of physical, cognitive and emotional impairment on function and determines the needs of clients through interviews, home visits, record reviews or consultation with appropriate agencies and individuals.
Assumes a case manager role to provide supportive/directive counseling to clients and families.
Refers, determines eligibility for a variety of agency and external services to meet financial, psychological, emergency care, emotional, or other needs, authorizes and/or coordinates services as necessary to complete individual service plans. Follows through with case management services to assure that services are provided in a timely and complete manner.
Coordinates with In-Home Aide sub-contractor, reviews billing and in-home aide task/time sheets for accuracy in a timely manner prior to submission to the finance department for payment.
Maintains a current record of case activity in the Compass document management system through timely documentation and scanning of pertinent documents.
Maintains a current record of time and case activity in the North Carolina Department of Social Services Day Sheet System.
Enters and maintains client records in the Aging Resource Management System (ARMS).
Adheres to the policies, rules, and regulations of the North Carolina Division of Aging and Adult Services Home and Community Care Block Grant. Knowledge, Skills, and Abilities Knowledge and understanding of social work practices and theories.
Understanding of the availability and interactions of a variety of community services and agencies and of a range of service providers.
Knowledge of medical terminology and disease processes and behavior management techniques.
Ability to make accurate assessments/evaluations and use analysis and judgment to develop an appropriate service/treatment plan.
Ability to utilize agency procedural guidelines in accomplishing the work.
Detail oriented, organized and able to work independently from home and maintain timely contact with agency-based supervisor.
Employees may experience emotional stress due to the nature of interactions with clients or service providers.
Minimum Training and Experience Requirements:
Graduation from a four-year college or university with a degree in human services, social work, or related field and moderate experience in human services, or an equivalent combination of education and experience.
