Effectively uses professional skill, objectivity, and insight. Uses advanced clinical training, insight, and experience to interpret da and to identify viable trement options. Assesses high risk factors, acuity, and need for services Independently evalues the client’s situion, including the veteran’s reaction and ability to deal with it, and arrives a reasoned conclusion. Based on the psychosocial assessment, uses professional judgment and advanced practice skills to make a psychosocial diagnosis. Develops psychosocial trement plans in coordinion with interdisciplinary team members, including goals for psychosocial clinical trement. Using known available resources and the initial assessment of the veteran’s likelihood to accept differing types of assistance, make initial and continuing decisions regarding use of VA and non-VA services and referrals. Arrives a reasoned conclusion as to the preferred course of agency action. Provides independent consultion and makes recommendions to interdisciplinary team on course of trement. Coordines screening of pients for specific mental health programs and provides ongoing trement per mental health program guidelines Is responsible for clinical Social Work practice in the area or program to which assigned, including promotion of Social Work services as a key component of trement. The social worker conducts timely assessment of -risk Veterans in crisis to identify immedie needs, evalue risk, and initie safety plan as approprie. The social worker provides interventions independently with Veterans and their families/caregivers who are experiencing a wide range of compliced medical, behavioral health, financial, legal, and psychosocial problems. They provide a range of interventions and trement modalities which may include individual, group, and/or family counseling or psychotherapy. They independently formule and implement a trement plan including measurable, achievable goals identifying the Veterans’ needs, strengths, weaknesses, coping skills, and psychosocial acuity. Social workers serve on committees, work groups, and task forces the facility and VISN level or in the community. They provide subject mter consultion to colleagues and students on the psychosocial trement of Veterans offering professional opinions based on experience, expertise and role modeling effective social work practice skills.
