Primary Purpose:
- The primary purpose of this position is to provide psychosocial screening to identify problems using a psychosocial component risk screening form and to assess, manage and follow-up psychosocial conditions identified through the psychosocial screening. Primary work days for this position are Tuesday and Friday 8:00 am
- 5:00 pm.

Essential Functions:
- Screen high risk prenatal clients receiving services through Craven County Health Department and complete a psychosocial assessment as indicated to identify problems and/or potential problems that may interfere with a healthy pregnancy outcome
- Develop and document a plan of care for each identified psychosocial problem
- Facilitate client in accessing appropriate services within the community such as WIC, Medicaid, PerinatalUnit for substance abuse, and establishing a medical home
- Follow-up to ensure client received the needed services
- Counsel high risk clients concerning their high risk factor and possible impact on their pregnancy and assist in obtaining necessary services and supplies
- Maintain good rapport with other agency staff as well as other service providers in the community

Minimum Education and Experience:
- Master’s degree in social work from an accredited school of social work; Bachelor’s degree in social work from an accredited school of social work and one year directly related experience
- Master’s degree in a human services field and one year of directly related experience
- Bachelor’s degree in a human services field from an accredited college or university and two years directly related experience
- Bachelor’s degree from an accredited college or university and three years of directly related experience
- Current LCSWlicense from North Carolina Social Work Licensing and Certification Board required.
