What You’ll Do:
The Intensive In-home Coordinator will assist in the day-to-day coordination of the Intensive In-Home program and will provide brief behavioral and emotional services to children and their families within a community-based model. The coordinator is a member of a 3-person team, with a maximum case load of 12 families in providing such services; conducts skill-building activities, provides parenting education, intervenes during a crisis, provides on-call coverage within the intensive in-home team 24/7/365. Responds appropriately and timely to crisis situations; develops, reviews, and follows individual Person Centered Plans with the Child and Family Teams for each child on their caseload. Provides psycho education to children and families; facilitates effective step-down to less restrictive services and increased reliance on natural supports over time. Collaborates, coordinates and advocates with collateral agencies and natural supports in order to meet family and consumer needs in all domains. Services are provided in the client’s home, school, and community settings, under the direction of the Intensive In-home Therapist. Education and Experience This position requires the credentials of a Qualified Professional (QP) Two years of documented experience with the child/adolescent mental health population is required. Proficiency in Microsoft Office applications Must have a valid Driver’s License
