Vaccination Declination, as a condition of employment. SUMMARY Health Services Social Worker assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Works in close collaboration with healthcare professionals of the facility (geriatric psychiatrists, MDs, fellows, NP, dietitians, and nurses) for current resident evaluations and treatments to further their understanding of significant social, emotional, medical, and mental health factors underlying resident’s health problem. Leads the interdisciplinary team in Behavior Management Clinical Rounds to analyze behaviors that negatively impact the resident and develops clinical treatment plans that integrate pharmacological and psychosocial interventions. Develop and implement clinical interventions to meet the biopsychosocial needs of each resident. Counsels and provide psychosocial education to residents’ families on diagnoses, treatment plan, and overall welfare. Completes psychosocial history and conducts psychosocial assessment to help determine most appropriate treatment plan and level of care for resident. For new admissions, will collaborate with multidisciplinary team regarding residents so as to develop and implement most appropriate treatment plan for resident. When appropriate, make referrals to outside supports to address any additional biopsychosocial, spiritual, and financial needs of residents. Maintains appropriate documentation of assessments, diagnoses, and services in compliance with federal and state regulations. Must interact courteously, tactfully and professionally with managers, co-workers, residents, vendors, or associates to preserve productivity and morale.

EXPERIENCE

Bachelor’s degree (B. A.) from four-year college or university; majoring in social work or human services field (sociology special education, rehabilitation counseling and psychology), preferably with one to two years related experience and/or training; or equivalent combination of education and experience. Must be CPR certified or ability to obtain once hired. Full-Time

Some evenings and weekends may be required to meet the demands of the position.
