Job Summary :
Provides patient centered navigation services and care coordination across the continuum of care with a multidisciplinary team. Responsible for assessing, developing, implementing and evaluating effectiveness and quality of oncology navigation and psychosocial programmatic services. Coordinates the development of community outreach goals and objectives for the cancer program. Collaborates with community partners and provides community outreach and psychosocial activities. Interfaces and communicates on regular basis with program leadership, medical staff and other key stakeholders. Performs tasks requiring independent knowledge and judgment.
Qualifications :
Masters Degree in Social Work from an accredited school of social work or a Masters Degree in the Human Services field required. One year experience preferred. Oncology experience preferred. Computer experience including Microsoft Office applications. Considerable skill in dealing tactfully with patients, families, physicians, staff and community. Broad knowledge of community health concepts, services and resources. Must have strong organization skills with initiative, creativity, public speaking skills. Requires ability to work in a self-directed environment with the ability to network, organize and coordinate a patient care team.
