AuthoraCare Collective is currently seeking a Social Worker – Palliative Care. This is a full-time hybrid position Monday – Friday 8:00am – 5:00pm. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
Job Summary :
Responsible for providing comprehensive assessment of the social, emotional, and practical needs of the Palliative Care and other program’s patients and family members. Provides advanced counseling, education, and referrals to address patient/family issues of grief and loss, adjustment, care needs and end of life planning. Functions as a member of an interdisciplinary team of care in collaboration with external partners. Education/Qualifications:
BSW or MSW required. One year of experience in social work with experience in a mental health or medical setting is required.
Other:
Valid state-issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
Required Skills/Abilities:
Excellent verbal and written skills. Strong Computer competencies needed. Ability to act with integrity, professionalism, and confidentiality. Prioritizing and good organizational skills. Ability to concentrate with large volumes of work, handle pressure of deadlines, good judgment, ability to follow procedures, ability to work independently and with frequent interruptions. Provide effective counseling to patients and their families, communicate effectively with persons having highly diverse backgrounds and educational levels. Work effectively as a member of the interdisciplinary team. Excellent assessment skills to assess patient/family needs, develop goals and plans for intervention. Sensory abilities to complete a physical assessment and communicate with patient/caregiver, physician, and other members of the interdisciplinary group. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
