Social Worker Divine Home Care Inc 2928 Highway 401 Business, Raeford, NC 28376 Job Overview We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and guidance to individuals and families facing various challenges. This role involves assessing client needs, developing treatment plans, and facilitating access to resources that promote well-being. The Social Worker will work collaboratively with other professionals to ensure comprehensive patient care. Responsibilities Conduct assessments to identify client needs and develop individualized treatment plans. Utilize data collection methods to monitor client progress and adjust treatment plans as necessary. Maintain accurate documentation in accordance with ICD-10 standards. Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate care. Advocate for clients’ rights and assist them in navigating social services systems. Experience A Bachelor’s degree in Social Work (MSW) or related field is required. Previous experience in social work or patient care is preferred. Knowledge of data collection methods relevant to social work practice is an asset. Proficiency in using systems for documentation and case management is a plus. Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds are essential.
