Description:
Assists in providing services to patients, their families, or significant others who are in need of help with social, economic, or environmental problems resulting from, or related to their cancer diagnosis. Using creative communication strategies, the Oncology Social Worker / Navigator will assist cancer patients to anticipate, identify and overcome barriers within their journey through the healthcare system. The Oncology Social Worker / Navigator works closely with the members of the multidisciplinary oncology team, including patients, families, physicians, nursing staff, chaplain, dieticians, hospital financial counselor, other disciplines in the Iredell Health System involved in medical, radiation and surgical treatment of the cancer survivor. Assists the patient in understanding their diagnosis, coordinating their treatment plans, seeking assistance from various appropriate social services and assisting the patient in communicating their treatment plan with their family and caregivers, documenting all interactions and outcomes. Partners with a network of community agencies to meet identified psychosocial needs of the cancer survivor during and post treatment. Organizes and facilitates community support groups for the oncology community. Has full access to patient health information.
Shift:
Days, variable, 8:00 AM – 4:30
PM Requirements:
Baccalaureate Degree in Social Work required. License or certification in Social Work preferred. MSW or Master in Counseling preferred. Oncology certification preferred. At least one year of hospital social work is required. At least one year experience in counseling and facilitation of support groups required. Experience in Patient Navigation preferred. Understanding of principles and practice of social work. Must have working knowledge of community social and health agencies. Must have knowledge of human behavior and personality dynamics. BCLS certification. Essential Physical Requirements Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds.
