  • Independent Living supports community wellness by maintaining a relationship with each independent living resident, and assessing their needs regularly, referring them to resources and facilitating their movement within the continuum of care at The Pines as needed.

The Social Worker collaborates with internal team members and with resident families to optimize the well-being and security of residents when the need arises, including assisting with crisis situations and hospital stays. Experience and Skills Previous experience in a life plan community, skilled nursing or healthcare setting. Knowledge of social work principles, ethics, and practices. Effective communication, interpersonal and organizational skills. Ability to work effectively in a team environment and collaborate with interdisciplinary teams. Compassion, empathy, and a commitment to advocating for residents’ well-being. What would you do Develop relationships with residents, meeting with them at least bi-annually to determine their psycho-social needs. When appropriate based on assessment, refer residents to services, coordinating with family as appropriate. Perform crisis intervention with residents, ensuring resident safety by facilitating resources, family communication, and healthcare navigation. Support hospital stays by maintaining contact, securing transportation and other needed items such as clothing. Coordinate/Collaborate with the HC Admissions & Transitions Coordinator with transitioning residents from IL to AL or Nursing. Educate residents and families on aging, aging resources and the continuum of care services offered at The Pines. Act as a champion for Resident Rights; advocate for resident well-being and provide information and education to residents, families and team members on resident’s rights and responsibilities. Respond to complaints and grievances, by conducting investigations and making recommendations for appropriate action. Provide direct services to residents, including counseling and emotional support to address issues such as adjustment to illness, grief, placement, and end-of-life care. Collaborate with interdisciplinary teams, including chaplain, medical staff, therapists, nurses, and caregivers, to ensure holistic care for residents. Maintain an understanding of and relationships with high quality outside service providers who may be of assistance to our residents. Maintain accurate and confidential documentation of social work assessments, psychosocial assessments, interventions, and charting notes. Stay up to date on regulations and laws related to senior services and healthcare social work practices.
