Job Description

Social Worker 4.1 4.1 out of 5 stars 3015 Enterprise Drive, Wilmington, NC 28405 •This is a dual position that functions mainly in the Social Services Department but also had Admissions Department responsibilities and requirements. Northchase Nursing and Rehabilitation Center is currently seeking an experienced, high energy, compassionate and results driven Social Worker to join our team. JOB SUMMARY

The Social Worker is responsible for completing Social Services related tasks. Job responsibilities may include but are not limited to: Perform all Social Services tasks and duties for the facility. Build and maintain good relationships with residents and families. Be a resident advocate where the resident is the center of all we do. Ensure excellent & timely communication with residents, families, staff, and referral sources. Create positive resident outcomes. Respect others by treating them the way we want to be treated. Demonstrate kindness, compassion, honesty, and integrity at all times. Help create a culture of superior customer service. •Please note that this Job Summary outlines the general nature and level of work to be performed. The Admissions Coordinator is responsible for providing oversight for all admissions and marketing related tasks. Job responsibilities may include but are not limited to: Assist with all admissions and marketing for the facility. Ability to achieve facility goals and targets. Work closely and build/maintain good relationships with all referral sources. Be a resident advocate where the resident is the center of all we do. Represent the facility to community and civic groups. Ensure excellent & timely communication with residents, families, staff, and referral sources. Create positive resident outcomes. Respect others by treating them the way we want to be treated. Demonstrate kindness, compassion, honesty, and integrity at all times. REQUIREMENTS

Bachelor’s Degree, preferably in Social Work, Psychology, Gerontology or other related field.
