Collaborates with local pediatrician offices, WIC, CDSA, Randolph Hospital, etc. to Increase community awareness of the case management for children birth up to age five program and to receive referrals for care management services. To provide case management services to Medicaid-eligible high-risk pregnant women. Collaborates with Pregnancy Medical Homes, local hospitals, DSS, etc. to increase community awareness of the Case Management for high risk pregnant women program to received case management referrals.
Minimum Education and Experience:
Bachelor’s degree in social work from an accredited school of social work.
Special Comments:
Monday through Friday, 8:00 a.m. – 5:00 p.m. However, given the nature of case management and / or public health events, evenings and weekend work may be required at times.
