Job Description

The first primary purpose of this position is in the provision of care coordination services to Maternal Health patients, for the provision of Care Management for High Risk Pregnancy (CMHRP) program for ante-partum and postpartum patients. The second primary responsibility of this position is to serve as a care manager for the Care Management for At-Risk Children (CMARC) program.

  • Examples of Duties:
  • Provides comprehensive management services through care coordination to pregnant and postpartum women ages 12-45 who are receiving prenatal and postpartum care in the private and public sector.

Offering referral resources to patients such as Medicaid, providing in-depth assessments to determine their psychosocial, medical, nutritional, educational and financial needs.

Developing an individualized patient care plan within the Virtual Health system.

Responsible for monitoring, referring, and assessing all services within program guidelines and meeting the needs of all children and families in the Care Management for At-Risk Children (CMARC) program and ensuring services are provided.

Provides direct counseling and intervention to children and families.

Offers guidance through the CMARC Program using the CMARC Program Step by Step guidance, standardized plan and the CMARC Program referral form as a guide.

Provides advanced social work services in complex cases, as well as the application of family counseling in dealing with the family dynamics of patients in the clinics.

Performs other duties as assigned.

  • Typical Qualifications:
  • Bachelor of Social Work or Master of Social Work from a Council on Social Work Education (CSWE) accredited social work degree program; ORRegistered Nurse; ORBachelor’s degree in a human service field with 5 or more years of care management/case management experience working with the specific population of low income, pregnant individuals and/or children ages 0-5 years; ORBachelor’s degree in a human service field with 3 or more years of care management/case management experience working with the specific population of low income, pregnant individuals and/or children ages 0-5 years and has certification as a Case Manager (CCM preferred)AND candidate must possess and maintain a valid driver’s license•
