Social Worker (Child Health) 2.5 2.5 out of 5 stars Monroe, NC General Statement The Social Worker, under general supervision, provides professional social work services to clients in a variety of settings including local human service agencies, correctional facilities, hospitals, schools, medical facilities, patient/client homes, etc. Work involves evaluating the client’s situation and his/her ability to deal with it, developing a social history, psychosocial assessment, service plan and/or treatment plan and follow-up. Services may include placement in a residential facility; basic counseling, support, and reinforcement in areas such as death and dying, employment, vocational, medical, health, or behavioral issues. Employee addresses such problems as adjustment to illness or disability, placement into an institution, rest home or nursing facility; financial concerns; socialization issues of the socially handicapped or those debilitated by age or illness. They may provide information to and confer with schools, natural parents and foster parents on rehabilitation plans, behavior management, and other problem areas or needs. Employee may be involved in recruitment, evaluation, and training of foster parents and other care providers, provides care management services and coordinate with a multidisciplinary team, or provides care management services to pregnant Medicaid recipients; and may develop community or service resources and/or develop volunteer programs. In certain treatment settings worker participates on rehabilitation/treatment teams and provides the social work perspective. Positions at this level combine difficult services performed under supervisory guidance with relatively conventional services performed under more relaxed supervision. Public Health positions are responsible for preparedness activities in the event of a natural disaster, disease outbreaks, and other threats to the health of the public. Individuals in this role will be required to participate in preparedness response activities which may include mass vaccination clinics and sheltering. Essential Functions
POSITION SPECIFIC FUNCTIONS
Child Health Clinic Performs assessments to identify needs and establish an appropriate care plan. Assesses for Social Determinants of Health. Refers to resources for food, clothing, housing, and transportation. Engages families to close care gaps in key health areas such as prevention, maternal, and child health. Recommends strategies to improve the practice infrastructure to address non-medical drivers of health. Supports pediatric patients and their families through challenges by providing general counseling services, crisis intervention, mental health referrals, substance abuse support, and other key services. Performs additional duties as assigned. CMARC Program Reviews data reports received from Care Impact, VH and CMARC referrals from providers and community members to identify children birth to five who would benefit from care management based on the Standardized Plan. Evaluates priority risk factors identified by claims data analysis, hospital records and referrals, then contact the family of the child and evaluate for needs based on program guidelines. Initiates a Comprehensive Health Assessments on all clients determined to be at risk. Establishes processes to support care management based upon risk stratification guidelines outlined in Standardized Plan. Complete LSP and SWYC face to face as appropriate. Engages clients in care management services by making timely contacts as outlined in the Standardized Plan based on needs and identified risks. Develops and documents patient-centered care plans by addressing barriers and determining interventions based on client needs. Refers clients to local community resources to meet the specific needs and follows-up to evaluate outcomes. Supports Public Health in preparedness exercises. Performs additional duties as assigned. CMHRP Program Reviews data reports received from Care Impact and pregnancy risk screening forms received from OB providers and community members to help identify pregnant women who would benefit from care management based on Standardized Plan. Contacts clients who have priority risk factors as identified by claims data analysis, risk screens and referrals and evaluate for needs based on program guidelines. Completes face to face Comprehensive Health Assessments on all clients with determined risk factors. Establishes processes to support the care management based upon risk stratification guidelines outlined in Standardized Plan for Pregnancy Care Management. Engages clients in care management services by making timely contacts as outlined in the Standardized Plan based on implacability scores and identified risks. Develops and documents patient-centered care plans by addressing barriers and determining interventions based on client needs. Refers clients to local community resources to meet the specific needs and follows-up to evaluate outcomes. Performs additional duties as assigned. Crisis Assessment Performs assessments to identify needs to determine resources and available funding, if any. Meets with clients face to face, talks with them on the phone, interviews both via phone and face to face. Returns telephone calls answering client inquires on available resources. Takes referrals for Adult Daycare, Adult In-Home Aide Services and assistance via telephone as well as in person. Provides supportive/directive counseling to clients/families in addressing needs and resources. Refers clients/families to a variety of services, resources in the community as needed and available. Maintains a good rapport with community agencies and organizations. Collaborates as needed with partner agencies, community resources and organizations to serve/assist clients/families as needed. Manages a caseload of Representative Payee clients. Assumes a case manager role for Representative Payee clients. Assists Representative Payee clients with accessing needed and available services. Completes appropriate assessments, quarter reviews and payment authorizations for Representative Payee clients. Completes a full assessment on clients applying for 200%/Emergency Assistance (Work First Diversion) clients. Conducts home visits and completes follow-up appointments as needed. Documents direct client interactions, services development, referrals in accordance with agency requirements. Maintains case data and documentation through various computer and State programs. Utilizes agency guidelines, specific Federal or State standards and principles of a variety of support counseling techniques to provide services and resources to clients/families. Enters data and keys applications to meet requires State/County/Federal time frames. Performs other related duties as assigned. Guardianship Acts as representative for adult Guardianship services to which the agency director has been assigned. Acts as liaison between facility and/or residential staff and individual under Guardianship. Practices critical decision making skills as it relates to individuals under Guardianship medical care, residence, day to day activities, and overall well-being. Acts as Authorized Representative for applications and ongoing maintenance of public service benefits for individuals under Guardianship. Performs assessments to identify needs and establish an appropriate service/treatment plan. Provides supportive/ directive counseling to clients and families in addressing needs and in some settings behavioral approaches are used to maximize client involvement. Serves as inter/multi-disciplinary team member. Refers clients and families to a variety of services to meet financial, psychological, emergency care, emotional, or other needs. Assumes a case manager role to follow the client through agency and external services. Monitors Adult Services caseloads and completes Quarterly and Annual Assessments. Completes annual status report of duties to submit to Clerk of Court. Provides transportation to/from interviews, placement locations, and doctor’s appointments. Submits receipts and payment requests daily, weekly, and monthly. Monitors and documents Adult Care Homes (ACH) compliance with licensure standards. Investigates complaints in ACH about general conditions or violations of standards. Reports violations of Residents’ Bill of Rights with the involvement of the N. C. Division of Health Service Regulation (NC DHSR). Documents direct client interactions, services development, and referrals in accordance with agency requirements. Utilizes agency guidelines, specific federal or state standards, and principles of a variety of supportive counseling techniques to provide services to clients. MINIMUM QUALIFICATIONS
Education and Experience:
CMARC /CMHRP/Child Health Programs Bachelor’s degree in Social Work with one (1) year of social work experience.
Licenses or Certifications:
Must have a valid Driver’s License and be able to maintain a safe driving history as defined by the Union County Vehicle Use Policy.

In order to perform work in this role, within (30) thirty days of employment you must provide immunization records, provide proof of immunity, or receive/start the required vaccination series, as applicable, along with complying with any necessary screenings, as required by the Union County Division of Public Health and its Healthcare Personnel Immunization and Tuberculosis Screening Policy, in accordance with applicable law. PHYSICAL DEMANDS
Must be physically able to operate a variety of machinery and equipment including computers, copiers, telephones, etc. Must be able to exert up to 30 pounds of force occasionally, and/or a negligible amount of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Light Work usually requires walking or standing to a significant degree. However, if the use of arm and/or leg controls requires exertion of forces greater than that for Sedentary Work and the worker sits most of the time, the job is rated for Light Work.

Work is performed in an environment where errors on the social worker and/or client’s part can lead to significant physical or mental consequences for them or others. Positions assigned to Public Health, work in performed in a dynamic environment that requires the need to the sensitive to change and responsive to changing goals, priorities, and needs.

LEAVE PROGRAMS VACATION LEAVE
Vacation leave is accrued based on creditable years of service in the Local Government Employees Retirement System.
SICK LEAVE
For full-time County employees, sick leave accrues at eight (8.0) hours per month. Benefitted part-time employees accrue prorated sick leave based on a percentage of the full-time rate. Your unused sick leave balance can be applied as creditable service toward a service retirement under the guidelines of the
NCLGERS. SICK LEAVE TRANSFER FROM OTHER AGENCIES
Unused sick leave earned from another North Carolina Governmental Agency and/or entity, will be accepted and transferred to the County as follows: twelve (12) sick leave days will be transferred and added to the employees active sick leave balance after completion of six (6) months of employment. The remaining balance of sick leave documented for transfer at the time of employment will be recorded in the employees permanent personnel file. On the fifth anniversary of employment with the County, the remaining balance of transferred sick leave will be added to the employees active sick leave balance.
HOLIDAYS
There are 12 County recognized holidays per year with 1 floating holiday available.
MILITARY LEAVE
Annual Training – 10 paid workdays granted. Active Duty – 20 paid workdays granted.
CIVIL LEAVE
Paid leave is provided to full-time or regular part-time County employees called for jury duty or as a witness of the court for the federal or state government, or a subdivision thereof.
FAMILY & MEDICAL LEAVE ACT
Federal law provides job protection and continuation of existing paid health insurance during 12 weeks of leave, paid or unpaid, to an eligible employee for qualifying events. These events can include birth or adoption, and serious illness of an immediate family member, or of the employee.
PAID PARENTAL LEAVE
Parental Leave provides the employee paid leave for the first eight weeks of approved FML for the birth of a son or daughter, or placement with the employee of a son or daughter for adoption or foster care.
ADMINISTRATIVE LEAVE
Educational Leave is available to regular full-time and benefitted part-time employees when participating in a job-related, short course, such as an update, not to exceed ten (10) consecutive working days.
VOLUNTEER TIME OFF
Volunteer Time Off will provide employees with 16 hours of paid time per calendar year to volunteer at approved Union County community partner agencies.
