(APS) Adult Protective Services -Social Worker 3.4 3.4 out of 5 stars Raleigh, NC 27629 Hiring Immediately!!! PIE Management is seeking an APS Social Worker in Raleigh, NC. This position will be responsible for conducting comprehensive psycho-social evaluations and assessments through onsite interviews with disabled adults and alleged perpetrators, telephone and personal interviews with collateral contacts.

Contract:

Position will go 6+ months in duration, with the possibility of a 6-month extension.

Job Description:

Provides ongoing case management upon substantiation of cases including, but not limited to, arranging for medical intervention, placement out of the home, home care, adult day care, pastoral care, meals, home repair, and financial intervention Develop and maintain treatment plans Referring to and facilitating services of mental health and substance abuse intervention Ongoing documentation of case progress to assess goal achievement Ability to manage crises interventions and negotiations with caregivers until client is no longer at risk Testifying in guardianship and other court hearings

Knowledge, Skills, Abilities, & Competencies:

Working knowledge of resources for elderly and disabled adults with mental illness, intellectual developmental disabilities and substance abuse Ability to prioritize and respond to needs of a large caseload Must be able to identify issues, obtain relevant information, relate and compare data from different sources, and identify alternative solutions Working knowledge of and ability to use effective approaches for choosing a course of action or developing appropriate solutions and/or reaching conclusions Ability to take action consistent with available facts, constraints, and anticipated consequences Able to read and understand legal, medical and mental health documents Collaborate and maintain effective relationships with others in order to encourage and support communication and teamwork Working knowledge of how to identify problems, determine possible solutions, and actively work to resolve the issues Must be relied upon to ensure that projects within areas of responsibility are completed in a timely manner Able to monitor programs and/or activities and take corrective action when necessary Ability to firmly adhere to codes of conduct and ethical principles Must have the ability to make decisions without regard to that personal prejudices, biases, and experiences
