Home Health Social Worker, MSW Location:

Cary/Durham/Raleigh, NC Job Summary PHC is a rapidly growing full-service Home Health Agency looking for an energetic and dedicated team member who is willing to learn new skills and improve our business processes. This is a fast-paced role in a growing company working in a very friendly and dynamic team. We are growing quickly and looking for dedicated Home Health professionals to staff Cary/Durham/Raleigh & Surrounding areas to provide excellent patient care. The Social Worker, MSW will be responsible for providing social work services to home health clients. The Social Worker, MSW will work with clients to assess their needs, develop treatment plans, and provide counseling and support services. The Social Worker, MSW will also work with other healthcare professionals to coordinate care and ensure that clients receive the appropriate services. Responsibilities Conduct assessments to determine clients’ needs and develop treatment plans Provide counseling and support services to clients and their families Coordinate care with other healthcare professionals to facilitate optimal patient outcomes Obtain physician orders for certification and recertification of home medical social work services Ensure changes in patients’ clinical status are communicated to other healthcare team members and agency office staff verbally and in writing in a timely manner Advocate for clients and their families to ensure they receive the appropriate services Deliver medical social work modalities and interventions appropriate to the patient’s clinical status and needs, and in accordance with the standard of care for patients’ age Provide, modify, or discontinue medical social work modalities and interventions based upon an ongoing assessment/reassessment of the patient’s clinical status and in accordance with physician orders Provide crisis intervention services as needed Identify risk of injuries or threats to life and health in the patient’s environment, i.e., environmental safety mobility risks and implements appropriate interventions to reduce those risks Maintain accurate and up-to-date client records, including: a. Timely submission of documentation b. Timely submission of daily invoices reflecting accurate hours worked Participate in case conferences and team meetings Provide education and training to clients and their families Address and supports cultural practices providing such practices do not harm the patient or others and do not interfere with the planned course of medical social work Stay up-to-date with current social work practices and trends, attend continuing education as required by company policy Maintain current knowledge of community resources. Identify and access appropriate external and internal resources to meet patient and/or family needs and to facilitate optimal patient outcomes while home health services are being provided, and for transfer and discharge process Utilize payer source manuals and home care licensure rules and regulations Comply with agency infection control policy Perform other duties as necessary as requested by the nursing director Qualifications Master’s degree in Social Work accredited by the Council on Social Work Education and has at least 1 year of social work experience in a health care setting Licensed as a Social Worker in the state of North Carolina Driver’s licenses and reliable automobile Experience working with individuals, families, and groups Prior home health experience preferred Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Ability to maintain confidentiality
