Summary:

To perform complex interventions relating primarily to the psychosocial and emotional needs of Veterans who have served. Provide case management services to Veterans and their families as they transition through the continuum of care at the NH Veterans Home.

  • YOUR EXPERIENCE

    COUNTSEach additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.

  • Minimum Qualifications:

Education:

Master’s degree in Social Work (MSW) from a college or university approved by the Council on Social Work Education.

Experience:

Three years’ experience involving psychiatric social work (mental, emotional or behavioral disorders) prior to or subsequent to obtaining the graduate degree.

License/Certification:

New Hampshire driver’s license and/or access to transportation with liability insurance as required by state laws if necessary for performing job accountabilities.

RECOMMENDED WORK TRAITS

Extensive knowledge of psychiatric case work principles, techniques and practices and their application to individual case problems. Extensive knowledge of mental health problems, of modern methods of treatment and of the social aspects of such conditions. Extensive knowledge of long term care, geriatric patients and associated behaviors/disorders. Considerable knowledge and skill in community organization and availability of community resources. Appreciation of involved psychological and environmental problems arising in connection with veterans and long term care facilities. Ability to exercise good judgment in evaluating situations and making decisions. Ability to write case histories and related reports. Ability to explain departmental policy, procedures and objectives. Ability to understand environmental factors in individual cases and to apply this understanding in social adjustments and rehabilitation. Ability to establish and maintain working relationships with representatives of other social agencies, institution officials, the general public, families and residents. Ability to communicate effectively. Must be willing to maintain appearance appropriate to assigned duties and responsibilities as determined by the agency appointing authority.

  • Responsibilities:
  • Completes mood, behavioral and psychosocial well-being sections of the MDS (Minimum Data Set) upon admission, quarterly and in the event of a significant change.

Provides clinical summary and/or discharge related documentation.

Completes psychosocial assessments on residents through in-depth co versations with the resident and their families. Assist in determining the needs of the resident. Uses diagnostic information to participate in formulation of treatment goals based on identification of problem area and develops interdisciplinary care plans and coordinates weekly care plan meetings.
