Job Description

Essential Responsibilities Provide case coordination and assuring continuity of services for patients. Complete a comprehensive needs assessment, develop a care plan, monitor efficacy of plan, provide resources, referrals, and advocacy. Monitor overall care of patients under assigned caseload and ensure that care is appropriate to need. Qualifications Bachelor’s degree in Social Work, Psychology, Human Services or other related field required.
