Position Overview:

Reporting to the Director of Social Services, the incumbent is responsible for providing social and emotional support/services to residents and families. They will complete psychosocial assessments and history for new admissions and will provide ongoing casework services to residents and families. The incumbent will make referrals and obtain services from the community in coordination with discharge planning services. They will assist with Medicare and Medicaid Benefits and will complete MDS and care planning assessments for caseload.

General Responsibilities:

The incumbent must have the ability to work as a team, independently, and be flexible. They must possess the ability to work effectively with residents, families, personnel and support agencies in a professional manner. The incumbent must have patience, a cheerful disposition and enthusiasm. They must be able to relate to work and work well with the ill, disabled, elderly, emotionally upset, dying and at time challenging people. They must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social service practices.

Qualifications:

Bachelor’s Degree – preferably in social work, human services, psychology or related field 2 – 5 years long-term and skilled experience in social services Must be proficient in computer usage, knowledge of electronic medical records, PCC and Mental Health Services preferable
