Our team is looking for someone who is capable of being compassionate and understanding toward our patients with unique needs. The Licensed Social Worker is an integral part of the interdisciplinary care plan team in providing insight into the psychosocial needs of our patients and their loved ones.

Qualifications:

The Licensed Social Worker or Licensed Clinical Social Worker is an individual licensed by the NJ State Board of Social Work Examiners and has a Master’s degree in Social Work from a graduate school of Social Work accredited by the Council on Social Work Education. At least one year of post-master social work experience in a health care setting in accordance with NJSA 45:1588-1 seq and NJAC 13:44G. Hospice experience preferred. Must be a licensed driver with an insured automobile in good working order.

Position Responsibilities:

Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to a hospice program. This includes emotional, social, financial, and environmental resources and identifying appropriate psychosocial problems, interventions, and goals for the patient’s plan of care. Participate in IDG meetings and the development of the patient’s plan of care. Observe, record, and report changes in the patient’s emotional and social factors that are being affected by the patient’s illness and his/her need for care, and his/her response to treatment. Assess the patient’s/family members’ ability to cope with the patient’s disease process. Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient’s health problems. Organize and lead bereavement group for community.
